Thank you for taking the time to contribute to this important project. We recognise that services are working under increasing pressures, and we greatly appreciate the time and effort involved in completing this survey.
The Service | Team Survey (left button) should be completed by a team representative who can provide information about your service‘s staffing, caseload, workload, and service delivery model.
The Individual Clinician Survey (right button) is intended for all clinicians and clinical support assistants to complete individually. Each clinician should submit their own survey, in addition to a single Team Survey completed on behalf of their service.
*To help us produce robust and meaningful recommendations, we would be grateful if you could refer to your service data where possible, rather than relying on memory or estimates.

Please read this privacy notice to understand how your survey data is stored and processed. It is important that you are informed and can agree before you complete the surveys
Preparing your answers | Please use service records, job plans, diaries or other available information wherever reasonably possible, rather than relying solely on memory. Accurate and sufficiently complete responses will strengthen ASCEND’s analysis and its recommendations for safer staffing, sustainable workloads and high-quality care.
Your confidential linkage code | The surveys do not ask for names, contact details or service names. Please follow the coding instructions in the Before You Begin guidance carefully. The code allows related team and individual responses to be matched for triangulation and may help ASCEND locate a submission if a correction or withdrawal is requested.
Completing the survey | Most questions are optional, allowing you to complete the information available and return to more detailed sections later. Every question nevertheless contributes to ASCEND’s aims, so please answer as fully and accurately as reasonably possible. After submitting, select Save my response to edit and keep the resulting link and your confidential linkage code securely.
