Surveys

Thank you for taking the time to contribute to this important project. We recognise that services are working under increasing pressures, and we greatly appreciate the time and effort involved in completing this survey.

The Scarlet Ibis, chosen as a symbol of resilience and positive change

The Service | Team Survey (left button) should be completed by a team representative who can provide information about your service‘s staffing, caseload, workload, and service delivery model.

The Individual Clinician Survey (right button) is intended for all clinicians and clinical support assistants to complete individually. Each clinician should submit their own survey, in addition to a single Team Survey completed on behalf of their service.

*To help us produce robust and meaningful recommendations, we would be grateful if you could refer to your service data where possible, rather than relying on memory or estimates.

Please read this privacy notice to understand how your survey data is stored and processed. It is important that you are informed and can agree before you complete the surveys

This step-by-step guide explains how to save your response for editing, allowing you to complete or amend the survey in stages. It also shows you how to retain a secure copy of your latest submission, helping to protect the work you have already completed.

Preparing your answers | Please use service records, job plans, diaries or other available information wherever reasonably possible, rather than relying solely on memory. Accurate and sufficiently complete responses will strengthen ASCEND’s analysis and its recommendations for safer staffing, sustainable workloads and high-quality care. Key resources: Before you start the survey, Important definitions, Completing your survey in stages.

Your confidential linkage code | The surveys do not ask for names, contact details or service names. Please follow the coding instructions in the Before You Begin guidance carefully. The code allows related team and individual responses to be matched for triangulation and may help ASCEND locate a submission if a correction or withdrawal is requested.

Completing the survey | Most questions are optional, not because they are not very important for the study, but to allow you to complete the information you have available when it suits you, return to add more detailed sections later and to move freely back and forth across the survey. Every question is required and contributes to ASCEND’s aims, so please answer as fully and accurately as reasonably possible. You create your own Base linkage codes within your team or service (please see linkage codes used to date). Individual clinicians are asked to add unique suffixes to separate as well as link all contributions as needed.

For the best experience, complete and review the survey on a desktop or laptop. Use the Back and Next buttons to move between sections; questions marked with an asterisk (*) must be answered before you can continue or submit.

Support | We want participation to be as straightforward as possible. If you have a question, encounter a problem or have a suggestion, please contact the ASCEND team through the Contact page and check the Project Updates page for current information updates.
